The following is a listing of some  forms that may be of use:

• 
  Generic Entry Form
• This is a generic form for any AKC event.  If you don't have the Premium, it may be used for any AKC Conformation or Performance Event

 

• Annual Dues Statement
• Dues Statement 2011
• Dues statement to accompany annual dues, with all contact information confirmed.
• 
  AKC ILP Form
  Do you have a rescue or a Cairn without AKC registration?  You can register them by using this AKC form and following the directions. With the ILP registration, your dog can take part in all AKC events, except Conformation (Show Ring), these include Earthdog, Obedience, Tracking, Agility and other AKC Performance events.  Give your dog something to do and they will love it.  
  

 

• CPR for Cairns
  ( and other canines, maybe some c*ts)
  Do you know how to do Doggie CPR? , Download this form and print it for reference.  
  

 

• Emergency Authorization Form 
  Do you wonder, what might happen to your dogs, if you were in a serious accident?
  
  Download this Word.doc and change the data to match your needs, and make sure someone knows where it is. 
  
  The second page is a sample, change it for your needs.
  
  If you wish Rescue to help with the dogs, specify that.  You can always consider assigning the dogs to the club. If your dog came from another breeder, you may want to include that contact information  

 

• Release of Ownership Form     
    Do you need to surrender a cairn to Rescue?  
  Fill out this form in advance and bring it along. You need to type in the boxes and then print it. 

 

•  CTCNC Adoption Agreement  
  When CTCNC places a dog through Rescue, we give Adopters a copy of this form and retain on copy of this form.  CTCNC members aiding in Rescue placement should use this form.
